Applying for a PCHF Grant

Paulding County Hospital Foundation’s mission is to support Paulding County Hospital by promoting high-quality health care and wellness for the people in our community. Over the decades, that mission has expanded beyond the walls of Paulding County Hospital.

PCHF Grants are available to non-profit groups or organizations focusing on improving the health and wellness of Paulding County. Funded projects must directly benefit the residents of Paulding County and support the mission of PCHF.

 

 

 To submit a grant request, a completed and signed application with all necessary supporting documentation must be submitted and received by the PCHF Executive Director by the deadline listed. Applicants are encouraged to contact the PCHF Executive Director for assistance in determining eligibility for a grant and working through the proposal process.

 Once approved, grant recipients will be required to return a completed Grant Acceptance Document before funds can be disbursed and will be required to submit a Final Report Narrative within ninety days of receiving the grant, or at the completion of the project or program for which the funds were received.

Paulding County Hospital Foundation Online Grant Application

  1. Grants are awarded twice per year:
    • June 1: Grant application deadline May 15
    • December 1: Grant application deadline November 15

    Grant recipients will be notified in writing of the final decision of the Grant Review Committee by the last day of June or December.

The following documents must be provided in order for your grant application to be considered complete.

  • An itemized list of estimated expenses for the project.
  • A copy of your IRS Determination letter, documenting proof of your organization’s 501c(3) status.
  • A list of your governing board members.
  • Two letters of support for your grant request.
